We Have A Single PCN Working For Over 260,000 Patients

When Primary Care Networks were introduced in 2019, the GPs in Barnsley had already been working in collaboration through Barnsley Healthcare Federation since 2015. As a result of our previous collaboration with Barnsley GPs, they could quickly adopt the new PCN structure and its benefits, reaching out to other providers and services across the Barnsley healthcare system.

Most boroughs consist of more than one PCN, but here in Barnsley we have a single PCN working for over 260,000 patients. This makes us the largest PCN in the country.

To make sure we’re meeting the diverse needs of our patients, we’ve organised the PCN into six neighbourhood networks:

  • Network one – Penistone
  • Network two – Central
  • Network three – North
  • Network four – North East
  • Network five – Dearne
  • Network six – South

Everything We Do Is Driven By Our Guiding Principles:

Each network focuses on community needs, working across GPs and other NHS and voluntary sector organisations to improve local health and wellbeing. We’ve introduced many new roles into GP practices and new ‘active signposting’ and care co-ordination, helping to take the strain off GPs and provide patients with access to the most appropriate care givers.

Our Guiding Principles:

  • Place patients at the centre of all PCN activity and decision making
  • Work together with a co-operative and positive approach
  • Adopt a solution-oriented method of addressing challenges that affect the PCN
  • Work collaboratively to manage and achieve the requirements set out by NHS England for PCNs (the Network Contract Direct Enhanced Service)
  • Share ideas, best practice and concerns with all PCN members, who work in a manner conducive to promoting effective working relationships
  • Act in an open and transparent way that promotes a sharing environment
  • Share and direct resources appropriately to enable the achievement of Network Contract requirements and improve the outcomes of patients in the PCN
  • Manage resources in a robust and effective way to ensure optimum service delivery for the local population
  • Work collaboratively with all local stakeholders to create an integrated care system in Barnsley that is capable of improving patient outcomes
